Flow Support is looking for an experienced and dedicated Senior Support Coordinator and Psychosocial Recovery Coach to join our growing team. This role involves working closely with NDIS participants to provide expert guidance, coordinate supports, and deliver psychosocial recovery coaching to empower participants in achieving their goals.
You will have the autonomy to manage your schedule while delivering face-to-face services to participants across the Greater Brisbane area, ensuring they receive the highest quality of care and support tailored to their individual needs.
Responsibilities
Deliver face-to-face support to NDIS participants in the Greater Brisbane area and provide remote support Australia-wide, focusing on complex and high-needs cases.
Assist participants in managing their NDIS plans, building their capacity, and achieving their personal and psychosocial goals.
Provide psychosocial recovery coaching to empower participants in improving their mental health, independence, and overall well-being.
Build and maintain strong, professional relationships with participants, providers, and stakeholders to ensure effective collaboration and outcomes.
Provide expert guidance and practical solutions to help participants make the most of their NDIS funding while navigating challenges.
Effectively prioritize and manage a diverse caseload, ensuring timely, high-quality service delivery tailored to individual participant needs.
Meet weekly KPIs aligned with your role, including participant engagement, reporting, and capacity-building outcomes.
Requirements
Location: Based in the Greater Brisbane region, with the ability to provide face-to-face support locally and remote support Australia-wide.
Experience: Minimum of 2 years in the NDIS industry, with a demonstrated understanding of NDIS processes, funding categories, and participant needs.
Licenses and Vehicle: Valid driver’s license and access to a reliable, insured vehicle (essential for local face-to-face visits).
Skills: Strong organisational and time management skills, with the ability to work autonomously and manage competing priorities effectively.
Communication: Exceptional written and verbal communication skills, with the ability to build and maintain professional relationships with participants, providers, and stakeholders.
Case Management: Proven ability to manage complex and high-needs cases, implement effective solutions, and achieve participant outcomes.
